The CooperativeCare network is comprised of a wide variety of primary and
specialty care physicians. Whether you need a family doctor to take
care of the common cold, an orthopedic surgeon for a knee replacement or an
oncologist for aggressive cancer therapy, the members of the CooperativeCare
Regional Provider Network are available
and qualified to take care of you.
Our Provider Search directory lists doctors, hospitals, other providers and
services within the CooperativeCare network. You are eligible for maximum
coverage according to the terms of your health care plan when you receive
services from any of the providers listed in this directory.
If you are referred for services that are not available within the local
primary network, please request a referral to a CooperativeCare tertiary provider
from your local physician or contact CooperativeCare directly. (A tertiary
provider is a hospital or medical center that offers highly specialized services
that are not offered within the local network.)
