About Our Providers

When your health care coverage is through the CooperativeCare Regional Provider Network, you have ready access to a full range of primary and specialized medical care, plus diagnostic and support services.  When highly specialized care is needed that is not available from within the local network, your doctor may refer you to a CooperativeCare tertiary provider.  (A tertiary provider is a hospital or medical center that offers highly specialized services that are not offered within the local network.)

If you are referred for services that are not available within the local primary network, please request a referral to a CooperativeCare tertiary provider.  Failure to use the services of a CooperativeCare tertiary provider may require you to pay a higher portion of your medical bills, as defined by your benefits plan.  To clarify whether your referred tertiary provider is affiliated with CooperativeCare, please contact us or call (859) 239-2421 or (800) 843-2597.

If you choose, or are referred to a physician or hospital outside the network for services that are available in-network, you can expect to incur higher out-of-pocket expenses.

This directory lists doctors, hospitals, other providers and services within the CooperativeCare network.  You are eligible for maximum coverage according to the terms of your health care plan when you receive services from any of the providers listed in this directory.
